Score combines publishing and usefulness: ten points per published paper, plus the net votes readers gave it. Counting papers alone would reward volume, which is the opposite of what this publication is for.
Tiers are earned by reaching a threshold, and they are kept. Unlike a leaderboard position, a tier does not change because someone else published this morning — which is what makes it worth putting on a CV.
