Astrana incident exposed patient, employee, and provider information

HIPAA Journal's lead sentence says the material incident "exposed patient, employee, and provider information." The 8-K does not say that. It says the company "continues to assess whether, and to what extent, patient, employee, credentialed provider, confidential business and financial information, intellectual property, or other information may have been accessed, acquired, or exfiltrated." The only point toward patient data is the company's statement that it intends to make required notifications "including to impacted patients." As of 2 October 2026 there was no HHS OCR portal entry and no state attorney-general notice to confirm any category or count. The headline goes beyond the filing it reports on.
